    Your provider evaluates whether RPM is right for your condition—such as hypertension, diabetes, heart failure, COPD, or post‑hospital recovery—and enrolls you in our program.

    We supply the necessary equipment (e.g., blood pressure cuff, glucose meter, pulse oximeter, smart scale) and guide you through easy setup—no tech skills required.

    Take readings at home as directed. Devices automatically transmit data to our secure platform for real-time tracking.
  4. Clinical Oversight & Intervention
    Our care team reviews your readings, identifies trends, and, alongside telehealth visits, makes timely adjustments to your treatment.
